Hola Support team members are very experienced and knowledgeable of the NDIS processes and can help you navigate complex processes to help implement the funded supports in your Plan and Our employs highly skilled and experienced Support Coordinators who assist you in navigating the disability service system. Our Support Coordinators will assist you in identifying, developing, coordinating and accessing supports and services on your behalf and helps you to access supports and services on their own.
We will work with you in a Person-Centred approach and collaboratively develop a plan that we will monitor and report on the goals achieved.

- Assist you to understand your NDIS plan and funding
- To provide information, advice and support when decisions are to be made
- Hep you monitor the progress and outcomes of your goals
- Guide you on how to access the NDIS portal so that you can monitor how your funds are spent

Difference between Support Coordination and Specialist Support Coordination?
A Support Coordinator and a Specialist Support Coordinator are very similar however there are some key differences.
A Specialist Support Coordinator will be funded where there are additional high or complex needs
Specialist support coordinators will support you to manage challenges in your support environment which may include health, education, or justice services.
Can Self / Plan Manage Participants be eligible for Support Coordination?
Yes. The decision for a plan to be self-managed, plan-managed or NDIA-managed has no effect on funding support coordination.
Role of a Support Coordinator
- Support the participant to understand and implement the funded supports in their plan and link them to community, mainstream and other government services.
- Focus on supporting you to build skills and direct your life as well as connect you to providers.
- Assist to negotiate with providers about what they will offer you and how much it will cost out of the approved plan.
- Ensure service agreements and service bookings are completed.
- Assist to build your ability to exercise choice and control, to coordinate supports and access local community.
- Assist Participants in planning ahead to prepare for Plan review meetings.
- Assist the participant to ‘optimise’ their plan ensuring that they qre getting the most out of funded supports.
